How to Register with Us

Online Registration is now available

Registering with the Practice is a simple and efficient process which can be done by clicking on the QR Code or scanning the QR code with your smart phone and completing the online registration form:

Alternatively, if you are unable to register online, please contact Reception who will provide you with the necessary forms and Practice Leaflet. In order that we can establish your current health all newly registered patients should make an appointment with the Practice Nurse for a New Patient Health Check and with a Clinician, if on current medication as it normally takes a few weeks for your previous medical record to be forwarded to us.

Once completed, you may:

  • post this to the Practice
  • hand it in at the Practice
  • or send a copy via email to [email protected]

If you require any assistance in completing these forms, please contact a member of the Reception team who will be able to assist.

If you live within the allocated Practice boundary, you will be registered as a ‘Dispensing’ patient. If you do not live within the Practice allocated boundary you may be able to register as an ‘Out of Area’ patient. Dependant upon your location of residency the Practice may, or may not, be able to register you for Dispenerary services.

As part of the registration process, a ‘New Patient Health Check’ will be required.

Temporary Residents

We are always happy to see any relatives or friends staying with you if they need a Doctor or Nurse. They will need to fill out a short form for the information to be returned to their own Doctor. Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms.

Emergencies

During Surgery hours an emergency is a medical problem that needs attention within 30 minutes. After hours it is a medical problem that cannot wait until the surgery opens.

Accessibility

The practice has same level disabled parking with slope access to the main building with dual handrails and automatic entrance doors (inner doorway width 78cm). For further details on accessibility provision click the ‘Accessibility Needs’ above.

Language

Please contact the surgery if you believe yourself or a family member may need the assistance of an interpreter at a consultation. The Practice will endeavour to obtain an interpreter if one is not already available.
